Areas of concern identified at Mayo mental health unit

St Anne's unit was the subject of an inspection report.

A Mental Health Commission (MHC) inspection report highlighted a number of issues of concern at a Mayo mental health centre.

St Anne’s Unit at the Sacred Heart Hospital in Castlebar was inspected.

One critical risk non-compliance with the regulation on risk management procedures, and four high-risk non-compliances with the regulations on individual care planning; therapeutic services and programmes, premises, and staffing were identified at St Anne’s.

The inspection was unannounced and occurred over four days in March 2024.

The inspection team were concerned that no physiotherapist was available to the approved centre as there had been a number of falls some resulting in serious injury. A falls prevention committee had commenced in January 2024 in response to the increased number of falls and a need for a physiotherapy service was documented but this need was not escalated to the risk register. Incidents recorded and risk-rated on the National Incident Report Form (NIRF) identified a number of resident falls and medication errors but the incidents were not reviewed with relevant managers, nor was an action plan developed to eliminate, reduce or manage the risk of such incidents as per risk management policy. The inspection team were concerned that resident safety was compromised due to risk management procedures which did not actively reduce all identified risks 

Comments within a residents questionnaires were positive and included feedback in relation to comments that the approved centre felt like a home from home.
