Aoife, 16, died at University Hospital Limerick (UHL) in 2022 after waiting for 12 hours to be assessed for suspected sepsis.

The Minister for Health said he wanted to speak directly to the 16-year-old’s parents on the subject.

HSE chief executive Bernard Gloster said that accountability was being pursued ‘lawfully’ and ‘fairly’.

Bernard Gloster said the accountability process had begun in relation to ‘several people’.
